I've done a little digging and haven't found much solid information. No code doesn't help.

Can these engines go out of time? TLDR: The midrange International 466 engines can have this happen. A 5/32 dowel pin indexes the gear on the crank (press fit) that drives the entire gear train. Shock load can shear the pin, moving the timing. The result is exactly this. Won't cold start without a snort of bang juice. Once warmed up, it will restart fine for the rest of the day. It runs fine. Often, there is no code if it hasn't slipped much or is older with a less capable ECM. Rare problem that can be a real puzzle if you haven't seen it.

Not likely here either, but I don't know enough about the Cummins gear train to know if this is possible. Plus, how could changing the starter cause that?
